I have cobbled together all the suggestions and presented them in a way that I hope meets all your requirements and can be downloaded fairly easily. Please take time to read the instructions below - it may help in the download.

You can either download the form by saving it as an HTML document or Print the page and then photocopy the result to your size requirements.

To print the page just go to each document and print each page. Note that the Authority to undertake Directed Surveillance is three pages long and the Renewal is two pages. Print the first page and then double click on each page to bring up the next and then print that page and so on.

Alternatively bring up each page and click FILE in the menu and then SAVE AS...you can then save as an HTML file for loading into your word processor package. After that your on your own although I might answer phone calls for the usual fee.

There are three forms I have prepared so far:

Authorty to undertake Directed Surveillance;

Renewal of Authority to undertake Directed Surveillance; and

Cancellation of Authority to undertake Direceted surveillance.
